Maryland requires employers to report, within 20 days, a new hire's name, address, date of hire and SSN to the Maryland New Hires Registry, 410-347-7524, using New Hire Registry Reporting Form.

Massachusetts
requires employers to report, within 14 days, a new hire's name and SSN to the Department of Revenue, 617-577-7200 ext. 30488, using Form W-4 or equivalent.

Michigan requires employers to report, within 20 days, a new hire's name, address and SSN to the New Hire Operations Center, 517-373-3190, using form Michigan New Hire Reporting for 3281, MI W-4 or equivalent.

Minnesota requires employers to report, within 15 days, a new hire's name and SSN to the Department of Human Services, Child Support Enforcement Division, 800-672-4473, using Form W-9, W-4 or equivalent.

Mississippi asks employers to report, within 15 days of hire, a new hire's name and SSN to the Benefit Payment Control Office, Employment Security Commission, 601-359-4885, using Form W-4 or equivalent.

Missouri requires employers to report, within 20 days of hiring, a new hire's name and SSN to the Department of Revenue, 800-859-7999, using Form W-4 or equivalent.

Montana requires employers to report, within 20 days of hiring, a new hire's name, address, SSN, and date of hire. Reports are to be filed with the Department of Public Health and Human Services, 406-442-7278, using form W-4 or equivalent.

Nebraska requires employers to report, within 20 days of hiring, a new hire's name and SSN to the Department of Health and Human Services, 402-471-9160, using Form W-4 or equivalent.

Nevada requires employers to report, within 20 days of hiring, a new hire's name, address and SSN to the Department of Employment, Training, and Rehabilitation, 702-687-4744, using Form W-4 or equivalent.

New Hampshire requires employers to report, within 20 days of hiring, a new hire's name and SSN to the Department of Employment Security, 603-271-4427, using Form W-4 or equivalent.

New Jersey requires employers to report, within 20 days of hiring, a new hire's name, address, date of birth and SSN to the Office of Child Support, 609-588-2355, using Form W-4 or equivalent.

New Mexico requires employers to report, within 20 days of hiring, a new hire's name and SSN to the Child Support Enforcement Division, 505-827-7728, using Form W-4 or equivalent.

New York requires employers to report, within 15 days of hire, a new hire's name and SSN to the NYS Department of Taxation and Finance, 800-972-1233, using Form W-4 or equivalent.

North Carolina requires employers to report, within 20 days, a new hire's name, address, SSN. Reports must be filed with the Department of Human Resources, 919-571-4114 ext. 304, using form W-4 or equivalent.

North Dakota requires employers to report, within 20 days of hiring, a new hire's name and SSN to the Child Support Enforcement Agency, 701-328-3582, using Form W-4 or equivalent.

Ohio requires employers to report all new hires within 20 days. Must report the new hire's name, address, SSN, date of hire and date of birth, to Ohio New Hire Reporting Program, 888-872-1490, using Form W-4 or equivalent.

Oklahoma requires employers to report, within 20 days of hiring, a new hire's name and SSN to the Employment Security Commission, 405-522-2550, using Form W-4 or equivalent.

Oregon requires employers in certain industries to report, within 14 days, a new hire's name, address, and SSN to the Department of Justice, Support Enforcement Division, 503-373-7300, using Form W-4.

Pennsylvania requires employers to report, within 20 days of hiring, a new hire's name, address, date of birth, date of hire and SSN to the New Hire Reporting Program, 717-787-6466, using Form W-4 or equivalent.

Rhode Island requires employers to report, within 14 days of hiring, a new hire's name and SSN to the Rhode Island Child Support Services, Department of Human Services, 401-277-2302, using Form W-4 or equivalent.

South Carolina, effective 10/1/98, will require employers to report, within 20 days of hiring, a new hire's name and SSN to the Child Support Enforcement Division, 800-768-5858, using Form W-4 or equivalent.

South Dakota requires employers to report, within 20 days of hiring, a new hire's name and SSN to the Department of Social Services, 605-773-3641, using Form W-4 or equivalent.

Tennessee, until 10/1/98, has a voluntary program that asks employers to report, within 20 days, a new hire's name, SSN, position, and date work begins (or date on which employment was refused) to the Department of Employment Security, 615-313-4880, using Form DES-230.11C.

Texas, whose program becomes mandatory on 10/1/98, asks employers to report a new hire's name, address, SSN, date of birth, date of hire, and salary information to the Office of the Attorney General, Employer New Hire Reporting Program, 800-252-8014 option 5, using Form W-4.

Utah requires employers to report, within 20 days of hiring, a new hire's name, address, and SSN to the Department of Workforce Services, through contract with the Office of Recovery Services, 801-536-7850, using Form W-4 or equivalent.

Vermont, until 10/1/98, when its program becomes mandatory, asks employers to report a new hire's name, address, and SSN to the Department of Employment and Training, Office of Child Support, 802-241-2194, using Form W-4.

Virginia requires employers to report, within 20 days, a new hire's name, address, and SSN to the New Hire Reporting Center, 800-979-9014, using Form W-4.

Washington requires employers in selected industries to report, within 35 days, a new hire's name, address, SSN, and date of birth to Employer Reporting Control, 800-562-0479, using Form DSHS 18-468.

West Virginia requires employers to report, within 14 days, a new hire's name, address, SSN, and date of birth to the Child Support Enforcement Division, Employment Reporting Program, 800-835-4683, using Form ERP-1.

Wisconsin requires employers in selected industries to report, within 20 days, a new hire's name, address, SSN, date of birth and date of hire to Wisconsin New Hire Reporting, 608-264-7737, using Form WT-4, W-4 or equivalent.

Wyoming requires employers to report, within 20 days, a new hire's name, address, SSN, and (optional) date of birth and date of hire, to the New Hire Reporting Center, 800-970-9258, using Form W-4 or equivalent.
